FY 2027 ICD-10-CM Code Updates: What Every Coder Needs to Know Before October 1, 2026

The annual ICD-10-CM code update takes effect October 1, 2026, marking the start of Fiscal Year 2027. Claims coded with incorrect or deleted codes on or after that date face rejection. If you haven’t reviewed the FY 2027 changes yet, you’re six days out — here’s what matters most.

By the Numbers: FY 2027 at a Glance

CategoryCount
New ICD-10-CM codes190
Deleted codes30
Revised code titles4
Effective dateOctober 1, 2026

This is a lighter update than FY 2026, which introduced 487 new codes. But lighter doesn’t mean low-stakes — the 30 deleted codes create direct claim exposure for practices still using legacy code sets, and several new additions address areas that come up constantly in daily coding.

Key New Code Areas for FY 2027

Gender Identity and Transition History

The FY 2027 update adds expanded specificity in the mental health and social history chapters:

  • F64.A — Gender identity disorder, in remission
  • New Z87.890x personal history codes capturing prior transition-related diagnoses and procedures with greater granularity

These additions reflect documentation demands for Medicare Advantage and commercial payer audits that are increasingly requesting specificity on gender dysphoria and remission status.

Alphabetic Index Clarifications

R41.0 (Disorientation, unspecified) receives new Alphabetic Index terms, formalizing guidance that coders have been navigating inconsistently. The revision establishes R41.0 as the default for general disorientation when a more specific underlying etiology is not documented — reducing the “query every time” burden for inpatient CDI teams.

Expanded Specificity Across Multiple Systems

The 2027 update continues CMS’s pattern of adding clinical specificity to areas where payer audit pressure has grown:

  • Musculoskeletal and connective tissue conditions receive targeted additions supporting more precise laterality and staging documentation
  • Expanded personal history (Z87.x) codes give CDI specialists better tools for capturing clinically significant prior diagnoses that affect current risk scores

The 30 Deleted Codes: High-Risk Exposure

Thirty codes are deleted effective October 1, 2026. Any claim submitted using a deleted code on or after that date will be rejected. Common exposure points include:

  • Saved favorites and code sets in your EHR — most systems do not automatically retire deleted codes from user favorites
  • Superbills and charge capture templates — paper and PDF templates rarely update automatically
  • Coding reference materials — printed code books from FY 2026 or earlier are now out of date

Best practice: Pull a list of your 50 most-frequently-used codes and cross-check against the deleted codes list before October 1. Prioritize any codes in categories that received significant structural changes.

HCC Implications for FY 2027

The FY 2027 code changes do not include a CMS-HCC model revision — CMS-HCC v28 remains in effect for the 2027 payment year. However, new codes and revised specificity can shift risk capture in meaningful ways:

  • More specific personal history (Z87.x) codes may support better documentation of conditions that carry indirect HCC value through their sequelae
  • Expanded mental health specificity can affect risk capture for dual-eligible populations
  • CDI teams working on Medicare Advantage should review the new codes in light of the conditions they most frequently document

What to Do Before October 1

If you code manually:

  1. Download the FY 2027 ICD-10-CM Tabular List from CMS.gov and load it as your reference on October 1
  2. Flag any deleted codes currently in your active templates or superbills
  3. Review the new codes in your most common body systems

If you use a coding platform:

  1. Confirm your platform has loaded the October 1, 2026 code set
  2. Test a search for one of the newly added codes (e.g., F64.A) to verify the update is live
  3. Check that deleted codes return an error or warning rather than resolving silently

If you’re a CDI specialist:

  1. Brief your physician documentation team on the R41.0 clarification — it affects query triggers
  2. Review the expanded Z87.890x personal history codes for documentation opportunities in your population
